Subject: Important Dates and Links for LGHS Athletics
Dear Los Gatos High School Families,
As we prepare for the upcoming school year, we want to ensure that all athletes are aware of several crucial dates and events related to athletic clearance and winter sports. Please take note of the following information:
1. Physicals for Athletic Clearance
Families should get a physical through their own healthcare provider or at any clinic that offers sports pre-participation physicals.
You can download the Physical Form here: Sports Physical Form.
2. Mandatory Heart Screening
All incoming Freshman athletes and those who did not complete the screening last school year must undergo a mandatory heart screening.
The date is Tuesday October 28th here on campus or you can sign up at another location of a different date
To sign up through the Kyle J Taylor Foundation, please visit:

Families may also complete screenings through their own healthcare providers. Proof of screening must be uploaded to your athletic clearance application.
3. Winter Sports Parent Information Night (Link for Parent Information Night Slides)
Winter Sports Parent Information Night will be held on Monday, October 27th at 6:00 PM in the Large Gym. This meeting is primarily for parents, but student-athletes are also welcome to attend.
We will begin with a general athletic presentation led by Athletic Director Kenny Perrotti, followed by sport-specific breakout sessions where families can meet coaches and learn details about tryouts, team expectations, and participation requirements.
All families with a student who plans to participate in a winter sport are strongly encouraged to attend this important meeting.
4. Athletic Clearance through Home Campus
Ensure you have completed your athletic clearance and uploaded your physical and heart screening proof (if applicable) via Home Campus.
Register now at: Home Campus
Please note that athletes will not be allowed to begin practice or tryouts until they have received clearance confirmation via email from Home Campus.
